Demonstrating the Value of PEDSnet EHR-Generated Data: Defining Key Determinants on the Path from Obesity to Asthma in Children (KOACH)

Project Summary: The degree and nature of the heightened asthma risk in obese children are unknown and require novel study approaches. Defining the incidence and determinants of asthma in obese children will equip families, clinicians, researchers, and policymakers with better information to guide lifestyle, prevention, and treatment decisions. The primary aim of this study will be to show that the PEDSnet Learning Health System can utilize longitudinal Common Data Elements (2009-present) to accurately and efficiently define key epidemiologic parameters and determinants (i.e., incidence, modifiers, and mediators) leading from pediatric obesity to asthma.

Study Design. Observational longitudinal study: retrospective cohort design using pre-specified cohort extraction criteria with a comparator group assembled using one-to-one participant matching

Engagement. Dr. Lang is the Director of the Nemours Children Hospital Asthma Center and has recruited a close-knit family advisory Working Group (WG) to assist in the Asthma Center’s development which has also included PCOR-related projects. The WG has met several times to discuss perceptions about asthma and barriers to improved patient-centered outcomes. The role of obesity in the recent epidemic of asthma, and obesity’s role in complicating asthma diagnosis and management were particular themes of these meetings and contributed to this proposal. The WG will continue to meet periodically to interpret the KOACH results and discuss how to disseminate and implement the findings so that KOACH findings are meaningful to families around the country.

Data Elements. We will pilot the data extraction process at our home data warehouse using only valid PEDSnet Common Data elements, then send the validated data query to PEDSnet which will ensure efficient use of PEDSnet time and resources.
